Whiplash is an acceleration/deceleration mechanism of transfer of energy to the neck and other parts of the body. This may be as a result from a rear end type of impact or side impact crash. The impact may result in either damage to the connective tissue injury (whiplash injury) or bony injury. A connective tissue injury (whiplash injury) is generally speaking damage to the ligaments, muscles, and tendons of the body, whereas bony injury is damage to the bones resulting in fractures, dislocations or other injuries. A "whiplash injury" can lead to a variety of other clinical manifestations (whiplash-associated disorders) AKA WAD.

The Whiplash event causes complex forces to act on the tissues of the spine: compression, bending, shear, and tension. These forces can cause micro-scopic tearing and in some cases visible tearing of the soft tissues of the spine. The most common injuries to the spine due to Whiplash are damaged spinal neck ligaments. These ligaments are the primary cause of many patient’s pain. The tissues that can be injured included:

Pain originating from the joints & ligaments of the neck can be felt from the back of the head into the neck, upper shoulders and the upper back or shoulder blade area. The neck has 7 cervical vertebrae which are called C1 to C7. Injury to the different parts of the neck (ligaments, discs, muscles) causes pain in different locations. Many times people will get referred pain into the arms, fingertips, shoulders, upper back, even into the jaw area. This pain is often times referred pain and is coming from the damage to your neck. Many times you can also get "contra coup syndrome" from the auto accident as depicted in the figure above. This is when your brain slaps & collides with the front of the skull and in the back of the skull. This can injure the frontal & temporal lobes and injures the occipital lobe, as well as the cerebellum, are injured. These injuries can manifest as loss of memory, forgetfulness, irritation, visual disturbances, and a host of other symptoms.
Common Medical Interventions
The most common Medical treatment for whiplash injuries usually consists of non-steroidal anti-inflammatory drugs (NSAIDS), muscle relaxants, and/or cortisone injections.
Whether prescribed by a doctor or purchased over the counter, these medications may initially help with pain, however when the drugs wear off, the pain usually returns sometimes worse than before. Many patients with chronic whiplash induced pain get no lasting relief from medications. The real big problem is the adverse risks associated with anti-inflammatory drugs such as stomach bleeding, kidney damage, liver damage, accidental deaths etc. as either a "side effect" from the drug, or toxicity of the drug. Drug-related deaths are not uncommon.
